Please do not download these forms and save them onto your computer for later use, as they are updated on an ongoing basis. Always download a fresh form from this web page.

Incomplete applications will not be accepted.

Prior to completing your application form ensure you have read and understood the relevant legislation, guidelines and policies which relate to your project.

Ethical approval is valid for 5 years, subject to submission of annual Progress Reports. On completion of the project a Final Report must be submitted.
If you require ethics approval for a project to continue after 5 years from the initial approval date, you must submit both a Final Report for the project and a new application for review. The 5 year limit to ethics approvals allows the Committee to fully review research in light of current legislation, guidelines and requirements.

The Ethics Secretariat sends automated annual report reminders when a report is due. This is to help researchers keep track of their reporting requirements. Annual report dates are noted in the Committee’s correspondence confirming final approval.
